Required for full-team members. Students agree to terms and conditions of being a team member. |
| Team 365 Hold-Harmless Agreement 2008-2009 |
Required for full-team members, both students and adults. Members agree to accept liability. |
| FIRST Hold-Harmless Agreement 2007-2008 |
Required for full-team members, both Students and Adults. Member agrees to accept liability. |
| Student Travel Permission Slip 2006-2007 |
Required for students traveling to Regional or National competitions. Gives permission from parents and school officials for time off of school to compete with team. |
| Sponsor Letter (1.8 MB) | To be given to potential sponsors -- provides information about the team and informs sponsors what their sponsorship level entitles them to. ***NOTE: High-quality, full-color copies are provided by the MOE Boosters. Print this out in color yourself only if you have a good printer and are willing to use a lot of ink!*** |
